Although our call volume sometimes requires that we place you on hold, we know you are busy and we respect your time. This online appointment request allows you to send us an email at your convenience.
Before sending us a request, please read these guidelines:
Do not use this request form for medical emergencies. If you are experiencing a life threatening emergency, please call 911. If you are experiencing a non-life threatening emergency, please call our office at 956-380-3441.
Please keep these in mind when sending sensitive information:
Your appointment request is being sent via an email message over the Internet. The Internet offers no guarantee of confidentiality for email messages.
Your employer may monitor email messages you send or receive while using a corporate communications system.
Your email message will be received by our business office, printed and referred to the appointments clerk. Our goal is to respond to your request within 24 – 48 hours. If you do not receive a response within 24 hours, please call our office.
